Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our family practice.  We will request that you complete our patient forms located in the New Patient Center on this site along with some additional forms depending on your situation.

This paperwork provides us with your health history, information on your condition and necessary confidential personal data for our records.


Consultation

Next, you will have a consultation with your doctor to discuss your health history, your health-related problems, concerns, goals and potential treatment options.

This initial visit is designed for your doctor to meet you, learn about you, listen to your concerns and expectations and  determine how chiropractic care can help you address your problems.


Examination

After your consultation, your doctor will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. This examination will allow the doctor to get vital information about how your complaints are related to the physical and neurological condition of your body.

The doctor will explain what is being done during the examination and nothing will be done in our office without your knowledge and consent.


X-Ray Studies

Depending on your specific condition, we may take X-rays as well. X-rays help us by giving us a picture of what is going on with your skeletal structure. We can view the bones, the disc spaces and the relative position of the vertebra and also may be alerted to any serious spinal conditions. Information gathered from analyzing your x-ray studies is a very important part of understanding your condition and developing an effective treatment plan for you. Most patients are amazed once they see their X-rays up close.

Viewing your x-rays can help you understand how conditions in your spine, misalignment or degeneration, are affecting your ability to function normally and contributing to the pain and disability that you are experiencing.

Report of Findings

Once the information is collected  and examinations are performed, your doctor will give you a detailed report of all findings and answer any questions including:

  1. What is going on with me?
  2. How can chiropractic care help me?
  3. What needs to be done to correct the problem?

After reviewing your health history, discussing your health goals, and examining your spine and X-rays, your doctor will review your individual treatment recommendations.

If there are aspects of your condition that require referral to other healthcare providers or additional evaluative studies, that will be discussed also.

Treatment

 At the conclusion of this initial  appointment, you can choose to  experience your first treatment. This may include: 

  • spinal adjustments
  • chiropractic physical therapy
  • stretching or exercise recommendations 
  • work on your soft tissues

The doctor will explain what is being done during the treatment process and you can ask questions about the different styles of treatment anytime.

The goal is to make the treatment process comfortable for you and to gain the most effective results.


Wellness Program

During this treatment phase of your care, your doctor may make some additional recommendations to address your condition. These recommendations may include:

  • ice or heat application instructions for control of pain and muscle spasm
  • certain activities or positions to avoid
  • at home exercises and/or stretches.

As your treatment at our clinic progresses your doctor may work with you to help you identify healthy habits and routines for your lifestyle.  Every person is unique, therefore everyone requires a  customized wellness plan. 

The purpose of our  chiropractic care and wellness program is for you to achieve good spinal alignment, establish a  healthy diet, develop an exercise routine to fit your needs and  maintain a positive physical and mental state.
